Thank you for applying to the 2010 Julian Brown Memorial Cup tournament presented by Charlotte United FC. Below is important information regarding Team Check-In.

Important Notes:

Each team must send a representative to check-in during one of the Check-In Sessions.
Pre-ordered t-shirts must be picked up at Check-In.

What to Bring to Check-In:

Below are the different types of teams attending the 2010 Julian Brown Memorial Cup. Please find the type of team that best illustrates your team and provide the required information at one of the team check-in opportunities:

US Club Teams

These are teams who are carded/rostered with US Club Soccer:

2009-10 US Club Soccer Coach & Player Passes with Photo (Signed, Stamped, Laminated)
Official US Club Roster (Original + 4 Copies)
Player Liability Release Form – Parent/Guardian Signature for Each Player!
US Club Medical Release Form - Each Player - Not Notarized
Hotel & Contact Information Form

Teams with Guest Players: See below!

North Carolina Youth Soccer Assoc. (NCYSA) Teams

NCYSA Rosters, Player Passes & Medical Release Forms are NOT Allowed to be Used!

These are teams who are carded/rostered with NCYSA:

Tournament Team Roster Form (Original + 4 Copies) - In Excel!
NCYSA Approval Form (see below) OR Proof of Age for Each Player (e.g. Birth Certificate, Passport, State Id (e.g. Drivers License))
US Club Medical Release Form - Each Player - Not Notarized
Player Liability Release Form - Parent/Guardian Signature for Each Player!

Teams with Guest Players: See below!

NCYSA Approval Form: The NCYSA will provide your team, upon request, with a Team Player Information document. The request must be submitted by a team rostered official by following the instructions below:

Go to NCYSA website www.ncsoccer.org
Click on Tournaments
Click on PTT (Permission to Travel)
Click on Online PTT
Proceed to ‘Team Information Form’ (near the bottom of the page)
Complete requested information
Click SUBMIT (there is no fee for the request)
In the drop down, choose "Request to attend a Non-Sanctioned Event".
Enter tournament information.
You will receive an email with the information to print off ‘team information form’ electronically

NOTE: Please do NOT request information on guest players from the NCYSA. See below for guest players.

Tournament Insurance: Charlotte United FC has secured supplementary insurance for tournament attendees. Your team does NOT need to apply for team insurance through US Club Soccer.

All Other Teams

These are teams who are non-US Club & non-NCYSA.
