Team Check-In

Thank you for applying to the 2013 Julian Brown Memorial Cup tournament presented by Charlotte United FC. Below is important information regarding Team Check-In.

Important Notes:

Each team must send a representative to check-in during one of the Check-In Sessions. THERE WILL BE NO OPPORTUNITY TO CHECK IN PAST FRIDAY EVENING!
Pre-ordered t-shirts must be picked up at Check-In.

What to Bring to Check-In:

All Charlotte United Teams

Refer to email from CUFC Tournament Director.

US Club Teams

These teams are REQUIRED to bring the following to team check-in:

Roster: US Club Roster (Must bring an original and 4 copies)
Medical Release Forms: US Club Medical Release Form (one for each player)
Player Liability Release Form: Parent/Guardian Signature REQUIRED for EACH PLAYER
Proof of Age: US Club Player Cards (one for each player)

All NCYSA Teams

These teams are REQUIRED to bring the following to team check-in:

Roster: Julian Brown Memorial Cup Team Roster Form (Must bring an original and 4 copies) – We will NOT accept NCYSA Rosters!
Medical Release Form: Julian Brown Memorial Cup Team Medical Release Form (one for each player)
Player Liability Release Form: Parent/Guardian Signature REQUIRED for EACH PLAYER
Proof of Age: NCYSA Team Information Form. The NCYSA will provide your team, upon request for a fee of $15.00, with a Team Information Form. The request must be submitted by a team rostered official by following the instructions below:

- Click on Tournaments & Events
- Click on Attending Non-Sanctioned Events
- Click the “Please Click Here” link at the bottom of the page
- Follow the instructions on the page

Birth Certificate, Passport, or State ID (i.e. Driver’s License)

Out of State Teams

These teams are REQUIRED to bring the following to team check-in:

Roster: Current Team Roster (Must bring an original and 4 copies)
Medical Release Forms: Current Medical Release Forms (one for each player)
Player Liability Release Form: Parent/Guardian Signature REQUIRED for EACH PLAYER
Proof of Age: Current Player Cards

All Miscellaneous/Non-Carded Teams

These teams are REQUIRED to bring the following to team check-in:

Roster: Julian Brown Memorial Cup Team Roster Form (Must bring an original and 4 copies) – We will NOT accept NCYSA Rosters!
Medical Release Form: Julian Brown Memorial Cup Team Medical Release Form (one for each player)
Player Liability Release Form: Parent/Guardian Signature REQUIRED for EACH PLAYER
Proof of Age: Birth Certificate, Passport, or State ID (i.e. Driver’s License)

Guest Players

Four Guest Players are allowed - teams must provide the following for each guest player:

Roster: Guest Player Name added to the bottom of the Julian Brown Memorial Cup Tournament Roster
Medical Release Form: Julian Brown Memorial Cup Team Medical Release Form, U.S. Club, or out of state medical release form
Player Liability Release Form: Parent/Guardian Signed Player Liability Release Form
Proof of Age: US Club Player Pass, Birth Certificate, Passport, or State ID

NC TEAMS: Please DO NOT request information on Guest Players form the NCYSA!

Tournament Insurance

Charlotte United FC has secured supplementary insurance for tournament attendees. Your team does NOT need to apply for team insurance through US Club Soccer.
